Schedule an appointment
In case you can’t contact (541) 923-7633 directly, you may opt to use our online Request an Appointment form to schedule a consultation. Our representative will contact you within 24 to 48 business hours from the time of your request and confirm the date and time of your appointment. Due to our patient-first care policy, we can see new patients on short notice in emergency cases or other special situations in which immediate care is needed. If at any time you are unable to make a regular appointment, please notify us at least 24 to 48 business hours in advance so we can reschedule your appointment at a more convenient time.
New-patient registration
Once your initial appointment has been scheduled, please complete the following forms beforehand to save your time in the office and for you to get into the doctor’s office immediately.

Please call our office if you have any questions as to your insurance coverage. We will be happy to verify this for you.
