1. One time charge for new patients only.

3. The examination at this appointment will determine which of the following club plans you are eligible for.
4. This fee is waived for existing patients.

Preventative Club:

1. $29.99/ month
2. Includes:
1. 2 standard cleanings per year
2. All necessary dental examinations
3. All necessary x-rays
4. 2 emergency evaluation visits, if needed
5. 2 fluoride treatments
6. 10% discount off of any additional treatment

Periodontal Prevention Club:

1. 3 periodontal visits per year:  $49.99/ month
2. 4 periodontal visits per year:  $59.99/ month
3. Includes:
1. Necessary periodontal cleanings
2. All necessary examinations
3. All necessary x-rays
4. 2 emergency evaluation visits, if needed
5. 2 fluoride treatments
6. 10% discount off of any additional treatment

Additional Information:

• All monthly dues must be set-up for automatic charge to a credit or debit card.
• No waiting periods!  You are eligible for your first visit after the first monthly due is paid.
• You must qualify for one of the above plans.  Your individual plan will be determined by your dental hygienist and may require an initial evaluation appointment.
• Your dental health will be re-evaluated each year to determine if you still qualify for your enrolled plan.
• Patients with active periodontal disease may be required to complete comprehensive periodontal therapy before qualifying for one of the above club plans.  Prices will be determined at your initial examination.
